The Children’s Transition team will work with young people who:
The Transitions team work with young people and their parents or carers to complete a Transitions Assessment. The assessment will cover what support someone might need when they turn 18. The Children’s Transition Team works alongside the Adult Transitions Team. When a young person is at a settled point and/or support is in place, they will move from the Transitions teams to an Adult Locality Team. This is based on which area they live in.
There is no automatic referral process.
